- Chronic Disease Management Plan
Previously known as Enhanced Primary Care Plan. Please liaise with your GP who can prepare your child's Chronic Disease Management Plan. This provides a partial Medicare rebate for up to 5 sessions of allied health services over 12 months.
The NDIS provides support to eligible children with disability or developmental delay.
We are able to provide therapy support to self-managed and plan managed clients.

Your health fund may reimburse a certain amount of your therapy fee. Please contact your provider to check what therapies your fund will cover.
